Financial Considerations


Because our Forsyth County Programs are grant sponsored, we are able to offer them at very low cost to families.

Typically, a small deposit is collected ($50-$75) which is refunded to families who attend at least 9 of 12 sessions. The deposit amount can be adjusted for families in need.



If you would like to help bring the healthy family program to your area, you can print an information sheet for your physician, health care provider or hospital community health educator.

Health Care Providers in Forsyth County may refer interested families for participation by contacting us via email at referrals@healthyfamilyusa.org

In your email, please include the following information:

Child’s name, age, and gender
Parent’s name and contact information: address, phone number, email address.

We will contact the family with information about the next available group sessions. You may refer interested families to our website at www.healthyfamilyusa.org for more information about the program.

Referrals

The Healthy Family Adolescent Weight Management Program provides appropriate and effective intervention for overweight adolescents.

Most of the families who participate in our program were referred by their health care provider.


Criteria for Participation
1. Age Criteria:
Adolescents participating in the Healthy Family Program must be 10-17 years old. (If you have a patient in need who is younger than 10, contact us and ask about our “Parent Only” option).


2. Weight Criteria:
Adolescents participating in the Healthy Family Program must have a Body Mass Index that classifies them as “overweight” (BMI for age and gender>95%ile) or “at risk for overweight” (BMI for age and gender 85-94%ile). Adolescents with a BMI > 25 meet these criteria. Adolescents on the younger side of this range meet the criteria at lower BMI’s.

3. Family Participation:
Adolescents participating in the Healthy Family Program must have an adult who is willing to attend weekly meetings with them. Often this person is a parent or guardian with whom the child lives. Some participants have come with a grandparent/other relative, neighbor, or teacher. Any adult who can provide support and influence the child’s environment is acceptable.


Gender Information
The Healthy Family Program originated as a weight management program for adolescent girls and their families. Over the past year, modifications have been made to the curriculum in order to effectively serve adolescent boys and their families. We have programming available for girls and boys in the Forsyth County area. Girls and boys are served in separate groups.
