Age Criteria:
Adolescents participating in the Healthy
Family Program must be 10-17 years old. (If
you have a patient in need who is younger than 10,
contact us and ask about our “Parent Only” option).
2. Weight Criteria:
Adolescents
participating in the Healthy Family Program must
have a Body Mass
Index that classifies them as “overweight” (BMI
for age and gender>95%ile) or “at risk for
overweight” (BMI for age and gender 85-94%ile).
Adolescents with a BMI > 25 meet these criteria.
Adolescents on the younger side of this range meet
the criteria at lower BMI’s.

Family Participation:
Adolescents participating
in the Healthy Family Program must have an adult
who is willing to attend weekly meetings with them.
Often this person is a parent or guardian with whom
the child lives. Some participants have come with
a grandparent/other relative, neighbor, or teacher.
Any adult who can provide support and influence the
child’s environment is acceptable.

The Healthy
Family Program originated as a weight management
program for adolescent girls
and their families. Over the past year, modifications
have been made to the curriculum in order to effectively
serve adolescent boys and their families. We have
programming available for girls and boys in the Forsyth
County area. Girls and boys are served in separate
groups.
